New Patient Form prior to your first visit.
To set up an appointment with
Haymarket Physical Therapy & Chiropractic, please note the following:
- Obtain a prescription from your physician. (Some chiropractic patients may not require a referral.)
- If your insurance requires a referral from your Primary Care Physician (PCP), have your PCP's referral coordinator complete the referral to Haymarket Physical Therapy & Chiropractic.
- If you do not have insurance, a prescription from your doctor is still required for physical therapy due to Virginia law.
- After the prescription is obtained, call Haymarket Physical Therapy & Chiropractic at (703) 753-0974. Your insurance card and referral will be needed to schedule your first appointment.
- If you did not print out and complete a New Patient Form from our Web site, please arrive ten to fifteen minutes early for your first visit to complete the required paperwork
- Please bring your insurance cards and prescription. You will be presented with a written verification of your insurance limitations, exclusions, and required payments for co-pays and deductibles including your insurance percentage responsibility. You will also be presented with HIPAA forms to read and sign.
